How to Tell the First Signs of Presbyopia - What Separates It From Tired Eyes, and the First Step

Text on your phone is hard to read. Hold it further away and it focuses. Toward evening it blurs more.

The first question that comes to mind is whether presbyopia has begun. Many people brace against that word, because trouble at reading distance tends to land as the first concrete signal that aging has started. The same symptoms, though, also arise from tired eyes. Long screen work, too little sleep, dryness. Each of those makes close work hard to read.

The two call for different responses. Tired eyes respond to rest and to changes in the surroundings; presbyopia needs something that compensates for the refraction. So the first task is telling which one you have.

Watch Whether It Shifts With the Time of Day

The plainest difference is variation across the day.

Blur from tired eyes fluctuates. Mornings are fine and it grows stronger by evening. A break lightens it. On days off it does not bother you. Unmistakable fluctuation means fatigue accounts for a large share.

Change that comes with age fluctuates less. The same distance is hard to read morning and evening alike. Rest does not bring it back. Over months, the distance stretches out little by little.

In practice the two are often mixed. When fatigue lands on an age-related change already underway, evenings alone become extremely difficult. That calls for addressing both sides.

To observe it, keep a record for about a week. Look at the same text at the same distance morning and evening, and note whether you can read it. A wide gap points to fatigue as the larger element; a narrow gap points to age.

Reading Distance as a Measure

The other clue is the distance at which you read.

When you look at something nearby, a structure inside the eye called the crystalline lens alters its thickness to focus the image. The range of this focusing adjustment narrows with age, and short distances become harder to focus on.

So with age-related change, the readable distance recedes. Text you once read at 25 centimeters has to be held out to 35 centimeters. If you extend your arm more often, this change is taking place.

To check for yourself there is a simple method: bring newspaper or book print gradually closer and measure where it starts to blur. If that distance has clearly receded, the range of the focusing adjustment has narrowed.

The measurement shifts, though, with the day's fatigue and with the light. Measuring several times and reading the trend is more dependable. Reading better in a bright place happens because the pupil narrows and the depth of focus increases, and that too points to age-related change.

Why the Phone Shows It First

Most people notice the difficulty on a phone before a book. There are three reasons.

The first is the short distance. A phone tends to be held closer than a book, loading the focusing adjustment more heavily.

The second is the small type. To fit enough information onto the screen, text is sometimes set small.

The third is the length of use. Looking close without a break tires the muscle that drives the focusing adjustment. That fatigue pushes the age-related change forward into view.

Difficulty on a phone therefore appears both as an early sign of age-related change and as an outcome of fatigue. If enlarging the text solves it, you are on the fatigue side; if you still hold the screen further away, you are on the side of the focusing adjustment.

What to Have Checked at an Eye Clinic

Before judging for yourself and buying reading glasses off the shelf, there is value in one check at an eye clinic. The reason is that difficulty at close range is sometimes produced by a different cause.

Here are the points worth confirming. The state of the refraction (whether a change in myopia or astigmatism contributes), the range of the focusing adjustment, the condition of the eye surface (whether dryness creates the symptom), and the pressure inside the eye plus the state of the back of the eye.

In particular, when someone myopic feels that close range has become easier to see, or when the prescription has shifted abruptly, another cause can be involved. A change in the crystalline lens can also move the refraction, so having it confirmed is safer.

And if, alongside the difficulty, you have symptoms such as part of the visual field dropping out, straight lines looking warped, or vision in one eye alone worsening suddenly, see an eye doctor without letting time pass. These sit in a territory separate from a problem of focusing adjustment.

The Load You Carry by Leaving It Alone

Enduring close vision you cannot manage has effects beyond the eyes.

Because the muscle that drives the focusing adjustment keeps working beyond what it can sustain, pain deep in the eye, headaches, and tightness in the shoulders and neck appear. Posture also collapses as you try to see at a distance that does not work, which adds to the load.

Reading speed drops as well, so the work takes longer. Looking for longer tires you further, and the loop closes.

The response is to compensate for the refraction. Reading glasses, progressive glasses or contact lenses, glasses built for one working distance. The right choice differs with the use, so sorting out which everyday distances trouble you before you ask helps you find something that fits.

Knowing how age-related eye changes begin saves you from taking the blur of this period as abnormal. And reducing eye strain from screen work works just as well for people with an age-related change underway. The age-related element is something to compensate for; the fatigue element is something to cut down. Treating the two separately is the practical response.
